Volunteer Agreement: Policies and Guidelines
Thank you for agreeing to volunteer at Anathoth House’s food pantry. Your work will help to assist needy people in the VA MD DC area. We appreciate all the work that volunteers do for us and we hope to make this a satisfying and fun experience for you as well. These guidelines have been established to create a safe, productive and gratifying volunteer experience for everyone. Policies: Cooperation with the rules and guidelines that have been established will make your experience at the Anathoth House Food Pantry rewarding and safe. Failure to follow these rules could result in serious injury or termination from the Volunteer Services Program. All volunteers must enter and exit through the front door. Volunteers are allowed to tour the Anathoth House Food Pantry; however, a staff member must be present to facilitate the tour. Volunteers must stay in assigned work areas. Volunteers must be at least 18 years old. Volunteers under the age of 18 must have a written parental/guardian consent to participant in our programs. Smoking, radios, headphones, purses, bags, and other personal items are not permitted in the kitchen. Smoking is permitted outside and 100 feet away from the main building. Under no circumstances will any person be allowed to work in the kitchen while under the influence of alcohol or drugs. Although Anathoth House Food Pantry is a kitchen/commercial environment, you must dress appropriately and modestly at all times. Nothing that may be safety hazard will be permitted (i.e., loose fitting clothing or dangling jewelry, open toed shoes, sunglasses). No forms of harassment will be tolerated. Anathoth House is committed to providing a work environment where women and men can work together comfortably and productively, free from all forms of harassment, sexual or otherwise. Report any accidents or injuries to your Anathoth House coordinator. Fill out an accident report provided by your coordinator. Only trained Anathoth House Food Pantry staff will be allowed to operate power equipment. All kitchen traffic signs, safety signs, and signals must be followed. Do not block emergency exits, fire extinguishers, or any equipment requiring immediate access. In case of a fire in the kitchen or fire drill, immediately stop what you are doing and proceed to the nearest fire exit. No horseplay is permitted in the kitchen. Volunteers must report all accidents and injuries or unsafe conditions or practices to your supervisor. Any pre-existing medical conditions that would restrict a person from performing any physical activities must be reported and on file before working in the kitchen. Use correct lifting techniques at all times. Do not attempt to lift over 50 pounds without assistance. Use proper protective gear provided for various jobs, such as goggles and gloves. Due to safety and security, this is strictly enforced at all times. In return, Anathoth House agrees to provide: · Adequate job training · Maintain record of all volunteer hours and provide community service certificates when applicable · Provide references and/or confirmation of hours worked (with advanced notice) · It is our policy to ensure that each volunteer is accorded equal treatment and opportunity; an Anathoth House volunteer will provide further information on these policies on request. Volunteer Agreement I promise that I shall hold in confidence all information regarding callers or clients of Anathoth House, Inc. I will not violate the confidential relationships between Anathoth House its volunteers, clients and callers to Anathoth House. I will not remove from the office of Anathoth House any written records or copies thereof. I accept full responsibility for maintaining the confidential and private nature of all client records and information. I understand that I am personally responsible and liable for any violation of this agreement. The information provided in this application is voluntarily supplied and is accurate and correct to the best of my knowledge. I understand the information may be used and is disclosed for Anathoth House, Inc.'s purposes and that as an Anathoth House Volunteer, I will not be paid for my services. Anathoth House, Inc. is not obligated to provide placement, nor am I obligated to accept the volunteer position offered. Opportunities are offered without regard to religion, creed, race, national origin or sex. I waive and release any and all claims against Anathoth House and its staff for injury or illness which may result from my service to the organization. My claims remain waived even though liability may arise out of negligence or carelessness of the persons or the entity mentioned above. By submitting your application you consent to the above agreement. I, on behalf of personal representatives, my assigns, heirs, next of kin, and myself do hereby agree to indemnify and hold harmless the Anathoth House Food Pantry, its employees, volunteers or agents (the "Released Parties") from any and all claims or causes of action that may arise out of the performance of my assigned duties as a volunteer. I waive any right of action I have against the Released Parties in consideration of my participation as a volunteer for the Anathoth House Food Pantry. I acknowledge that this waiver and release is being signed by me voluntarily, without coercion, duress, or undue influence and with full knowledge of its terms and effects. I have read the above waiver and release of liability and fully understand its contents.
